CRF Library

# Form Builder

Browse, preview, create, approve and reuse CRF templates from the Templates and Category tabs of Form Builder.

Overview

CRF Library is a catalog of reusable CRF (Case Report Form) templates, organized by category. Study builders use an approved template as the starting point for a new CRF in their study instead of designing every form from scratch. The catalog is shared across studies: a template approved once can be reused in any study.

CRF Library opens from the Templates and Category tabs at the top of Form Builder, next to the Form Builder tab.

Template Status

StatusMeaningAvailable actions

ApprovedTemplate is released for reuseUse template, Preview, Archive, Delete
ArchivedTemplate is withdrawn; it can no longer be used or editedDelete

Approving a template archives the revision that was previously approved for the same template, so only one approved revision is offered for reuse at a time.

Browse Templates

  1. Open Form Builder and select the Templates tab.
  2. Filter the list by Status and Category, or sort by Name, Status, Created On or Last Updated On.
  3. Click Preview on a template to open a read-only preview of its fields.

Create a CRF from a Template

  1. On the Templates tab, click Use template on an Approved template.
  2. In the Configure form dialog, enter the CRF name and, optionally, a Description.
  3. Confirm the dialog. A new CRF is created in the current study from the template and opens in the Form Builder editor.

The new CRF is a copy: later changes to the template do not change CRFs already created from it, and changes to the CRF do not change the template. Adjust the CRF in the editor, then save and publish it as described in Build a Complete CRF.

Create a Template

  1. On the Templates tab, click New Template.
  2. Complete the dialog — the same one used to create a new CRF in Form Builder — and confirm. The template is created in Draft status and opens in the editor.
  3. Design the template fields in the editor and save.
  4. Back on the Templates tab, use Info to set the template name, description and Category tags.
  5. When the template is ready for reuse, click Approve and confirm. The template becomes available for Use template.

To change a Draft template later, click Edit to reopen it in the editor, or Info to change its name, description or category tags.

Archive or Delete a Template

  • Archive withdraws a template from reuse. A template can be archived at any time, including after CRFs have been created from it; those CRFs are not affected.
  • Delete removes a template permanently and cannot be undone. A template that has already been used to create a CRF, or that has been superseded by a newer revision, cannot be deleted — archive it instead.

Categories

The Category tab lists the categories used to organize templates, with each category's description. Assign categories to a template through Category tags in the template Info dialog, and use the Category filter on the Templates tab to find templates by category.
