Form Builder - Clinical Elements

# Elements

Reference for the Form Builder Clinical Elements — Blood Pressure, BMI, Vital Sign, and Prescription — including their auto-generated sub-field structure and configuration options.

Overview

Clinical Elements are a special category of Form Builder components. Unlike a Basic, Advanced, or Group Element — which places a single field or an empty container on the canvas — dragging a Clinical Element auto-generates a predefined, multi-field structure with the correct clinical grouping (and, for BMI, an automatic calculation) already wired in.

Not equivalent to hand-built fields

Building the same-looking fields yourself with plain Number fields (for example, three separate Number fields for Height, Weight, and BMI) does not reproduce the same behavior. Most importantly, a hand-built BMI field will not auto-calculate — see BMI below. Always use the Clinical Element when the CRF needs Blood Pressure, BMI, Vital Signs, or a medication list, instead of building equivalent-looking fields from scratch.

There are 4 Clinical Elements: Blood Pressure, BMI, Vital Sign, and Prescription.

Each generated sub-field can still be configured individually using the standard field settings (gear icon) described in Component Settings (Common) — label, code, required, read-only, PHI, help text, and hidden all apply per sub-field, the same as any other field on the canvas.

Blood Pressure

When to use: Use Blood Pressure to capture a systolic/diastolic reading as one logical pair, instead of adding two unrelated Number fields. Dragging Blood Pressure onto the canvas generates one column pair — Systolic and Diastolic — tagged together as a single clinical group.

Basic settings

Generated Sub-fieldData TypeUnitNotes
SystolicNumbermmHgUpper (systolic) blood pressure reading
DiastolicNumbermmHgLower (diastolic) blood pressure reading

There is no separate Advanced Configuration for this element beyond the standard per-field settings (for example, adding a Validation rule with a clinically acceptable range) available on each generated Number sub-field.

Who configures this

Adding and configuring Clinical Elements on a CRF is done in the Form Builder editor by Study Administrator or System Administrator.

BMI

When to use: Use BMI when body mass index needs to be captured from Height and Weight, and calculated automatically rather than typed in by the site user. Dragging BMI onto the canvas generates a row of 3 Number fields: Height, Weight, and BMI.

Basic settings

Generated Sub-fieldData TypeUnitRead-onlyNotes
HeightNumbermNoEntered by the site user
WeightNumberkgNoEntered by the site user
BMINumber—YesDerived value — see the auto-calculation rule below

Auto-calculation rule:

SettingValue
FormulaWeight / Height²
Referenced fieldsThe Height and Weight sub-fields generated in the same drop
TriggerRecalculates automatically whenever Height or Weight is entered/changed
BMI field stateRead-only — cannot be manually edited by the site user
This is what distinguishes BMI from three plain Number fields

The BMI sub-field ships with a real Calculation rule wired to the sibling Height and Weight fields generated in the same drop — this is automatic, not something a user configures afterward. A form built with three separate Number items (Height, Weight, BMI) instead of the BMI Clinical Element does not get this calculation: someone would have to manually type in a value that looks like a BMI. Always use the BMI Clinical Element when the CRF needs a body mass index value.

Vital Sign

When to use: Use Vital Sign to capture a standard set of vital sign measurements in one drop instead of adding four separate Number fields one at a time. Dragging Vital Sign onto the canvas generates a row of 4 Number fields, grouped together as one clinical tag (no calculation is applied — each sub-field is an independently entered value).

Basic settings

Generated Sub-fieldData TypeUnitNotes
TemperatureNumber°CBody temperature
PulseNumberbpmHeart rate
Respiratory rateNumber/minBreaths per minute
Oxygen saturationNumber%SpO2

There is no separate Advanced Configuration for this element beyond the standard per-field settings (for example, adding a Validation rule with a clinically acceptable range) available on each generated Number sub-field. Unlike BMI, Vital Sign does not wire any Calculation rule between its sub-fields — grouping/tagging only.

Prescription

When to use: Use Prescription when the CRF needs a concomitant-medication-style, repeatable table of prescriptions — for example, recording each medication a subject is taking with its dosage, route, and frequency. Unlike the other three Clinical Elements, Prescription generates a whole Table division (titled "Prescription"), not a row of standalone fields — each additional prescription is added as a new row.

Basic Configuration — pre-configured columns

ColumnData TypeNotes
MedicationShort Text (autosuggest)Suggests common medication names (for example, Paracetamol, Ceftriaxone, Metformin)
DosageNumber (decimal)Minimum value 0
UnitDropdownmg, g, mcg, mL, tablet, capsule, drop, puff, patch, unit
RouteDropdownPO, IV, IM, SC, SL, PR, Topical, Inhalation, Ophthalmic, Otic, Nasal
FrequencyDropdownQD, BID, TID, QID, QAM, QHS, Q4H, Q6H, Q8H, Q12H, PRN, STAT
DurationNumber (decimal) + unit selectorDuration value paired with a day/week/month unit selector
Special InstructionLong TextFree-text instructions (for example, "take with food")

Because Prescription is a Table division (see Group Elements), site users add one row per medication — the same repeat-row behavior as any other Table.
